This file is a Testing Request and Record / Work Inspection Request (WIR). It is used in engineering and construction projects to request specific laboratory or field testing for materials or executed works, and to officially document the findings and results.

Objective of the Report:

• Testing Coordination & Scheduling: To identify the specific item or work to be tested, its exact location, and to set the proposed date and time for the inspection in coordination with the testing party.
• Specifying Technical Requirements: To document the specific testing standards and criteria required for the work or material to pass, based on the project specs.
• Recording Field Results & Conditions: To provide an official section for logging the environmental conditions maintained during the test (such as temperature or humidity) and documenting the final test results and observations.
• Official Witnessing & Signatures: To involve all relevant parties (Consultant, Contractor, Supplier, and Project Manager) to sign as witnesses, validating the test results and ensuring quality control.
